  3. Statements of Good Faith Include a statement, under penalty of perjury, that you believe the material has been removed or disabled because of a mistake or misidentification.
  4. Consent To Jurisdiction Include a statement consenting to the jurisdiction of federal courts in your area and agreeing that you will accept service of process by the person who originally filed the DMCA notice.
